🎉 Parents Night Out at PSC! 🎉

As the parents/legal guardian of the child named above, I hereby give my full consent and approval for my child to participate in Parents Night Out. I understand that there are certain risks of injury inherent in attending PNO, as well as other related activities incidential to my childs participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my. child is fully capable of participating in PNO and my is healthy. In addition to giving full consent of my childs participation, I do hereby waive, release abd hold harmless the organizition below, its board of directors, management and lifeguards for any injury that may be suffered by my child in the normal course of participation in the designated activities incidental thereto, where the result of negligence or any other cause.
